3. Daily Monitoring
At House of Vibes Recuperative Care, our patients receive support-monitoring from our dedicated staff 24 hours a day, 7-days a week. Our trained staff is committed to helping our patients recover quickly and find the support necessary to prevent homelessness and reintegrate into the community.
4. Transportation
Transportation can be difficult to obtain for those facing homelessness. House of Vibes Recuperative Care provides prompt and reliable transportation to and from medical appointments to our patients.
5. Comprehensive Homeless Case Management
Our patients at House of Vibes Recuperative Care receive comprehensive homes case management as part of our standard treatment process. Our staff is well-prepared to provide referral between our patients and homeless case management professionals.
6. Behavioral Health & Addiction Prevention Services
Mental Health conditions and Addiction are some of the major barriers faced when in recovery or breaking the cycle of homelessness. House of Vibes Recuperative Care partners with other recuperative care programs and community support services to provide our patients with access to behavioral health clinics, AA meetings, methadone clinics and other services to prevent the cycle of homelessness.
7. Mental Health Assessments & Referrals
Our team of professional staff at House of Vibes, Recuperative Care includes specialized nurses and social case managers to help patients coordinate mental health care as apart of their recuperative treatment. Mental health is an important element in the path to breaking the path to homelessness. Our staff is dedicated to providing our patient with the mental support they need.
8. Public Benefits Management
As part of our standard at House of Vibes recuperative care, we help patients apply for and manage social service resources like state identification cards, Cal-Fresh, SSI, VA, and other California support programs.
9. Transitional Housing Assistance
House of Vibes Recuperative Care is dedicated to helping our patients live healthy, successful lives after leaving our facility. We maintain community relationships and connect patients with case managers who can assist our patients in accessing temporary housing, specialized service providers, board and care, charity programs, employment assistance and other programs.
